Dear Editor: Since the publication of Objective Assessment of Skills in Intraocular Surgery (OASIS) in 2005, we have received numerous requests from academic institutions and private surgeons both in the United States and abroad requesting inclusion in the OASIS database as well as updated templates to assess patient outcomes objectively. As we continue to establish a centralized database for all participating surgeons and institutions, we have made numerous changes to our initially published OASIS template (Fig 1 [available at http://aaojournal.org]). Many of the changes reflect new options and technologies now available in cataract surgery and needed data for future outcomes analysis. We continue to find the objective assessment of our surgical outcomes particularly helpful in the academic setting and hope more of our colleagues will join in collecting this information for the purpose of improving overall patient care.
